Insurance Company License - Issuance or Renewal

Pursuant to District of Columbia Code Sections 31-2502.02, 31-3402, 31-3403(g)(h)(i), 31-3902, 31-3903, 31-3904, 31-4102, 31-4103, 31-4304, and 31-5324 all licensed insurance companies and Health Maintenance Organizations (HMOs) must renew their Certificates of Authority annually by May 1. Please download, complete request for renewal and forward with appropriate fee to the designated address. New issuances must comply with the UCAA Application Process.
